How it works

Zoledronic acid belongs to the class of bisphosphonates and acts primarily on bone. It is an inhibitor of osteoclastic bone resorption. The selective action of bisphosphonates on bone is based on their high affinity for mineralised bone, but the precise molecular mechanism leading to the inhibition of osteoclastic activity is still unclear. In long-term animal studies, zoledronic acid inhibits bone resorption without adversely affecting the formation, mineralisation or mechanical properties of bone. In addition to being a potent inhibitor of bone resorption, zoledronic acid also possesses several anti-tumour properties that could contribute to its overall efficacy in the treatment of metastatic bone disease.

Side Effects

Severe kidney impairment , Low level of calcium in the blood , Pain in the mouth, teeth and/or jaw, swelling or non-healing sores inside the mouth or jaw, discharge, numbness or a feeling of heaviness in the jaw, or loosening of a tooth , Irregular heart rhythm (atrial fibrillation) , Severe allergic reaction: shortness of breath, swelling mainly of the face and throat , As a consequence of low calcium values: irregular heart beat (cardiac arrhythmia; secondary to hypocalcaemia). - A kidney function disorder called Fanconi syndrome (will normally be determined by your doctor with certain urine tests).

Drug Interactions

Aminoglycosides , calcitonin , loop diuretics or other calcium-lowering medicines, Thalidomide or any other medicines which may harm your kidneys. - Any other medicine that also contains zoledronic acid and is used to treat osteoporosis and other non-cancer diseases of the bone, or any other bisphosphonate, Anti-angiogenic medicines .

Indication

It is used: To prevent bone complications, e.g. fractures, in adult patients with bone metastases (spread of cancer from primary site to the bone) ; To reduce the amount of calcium in the blood in adult patients where it is too high due to the presence of a tumour. Tumours can accelerate normal bone change in such a way that the release of calcium from bone is increased. This condition is known as tumour-induced hypercalcaemia (TIH).

When not to Use

You should not be given Zoledronic acid: If you are breast-feeding, if you are allergic to zoledronic acid, another bisphosphonate (the group of substances to which Zoledronic acid belongs), or any of the other ingredients of this medicine .

Zometa Injection Precautions

Precaution

Standard hypercalcaemia-related metabolic parameters, such as serum levels of calcium, phosphate and magnesium, should be carefully monitored after initiating zoledronic acid therapy. If hypocalcaemia, hypophosphataemia, or hypomagnesaemia occurs, short-term supplemental therapy may be necessary. Untreated hypercalcaemia patients generally have some degree of renal function impairment, therefore careful renal function monitoring should be considered.

Zometa Injection Warnings

Warning 1

Patients must be assessed prior to administration of Zoledronic Acid to ensure that they are adequately hydrated.

Warning 2

Overhydration should be avoided in patients at risk of cardiac failure.

Warning 3

Osteonecrosis of the jaw (ONJ) has been reported uncommonly in clinical trials in patients receiving Zoledronic Acid. Post-marketing experience and the literature suggest a greater frequency of reports of ONJ based on tumour type (advanced breast cancer, multiple myeloma). A study showed that ONJ was higher in myeloma patients when compared to other cancers.
